Act as the first point of contact for patients experiencing a physical or mental health problem
Establish a long-term relationship with patients to support continuity of care
Perform clinical examinations to assess, diagnose and monitor the patient's condition
Interpret findings from investigations, such as blood tests, to assisting reaching a diagnosis
Consider each patient's current and previous medical history and the pattern of symptoms to assist In achieving a diagnosis.
Work with the patient to create a treatment management plan which may include a variety of treatment options such as counselling, prescribing medication, referring for further testing or a second opinion. Meet with wider multidisciplinary teams including nurses, midwives, health visitors, pharmacists, physician associates, psychiatrists and care of the elderly specialists to discuss individual patient cases and coordinate a person-centered care plan for the patient
Run clinics for patients with chronic conditions such as asthma, hypertension and diabetes, to ensure they have a well-rounded understanding of their condition and how best to manage it Prepare letters and reports to be sent to the wider multidisciplinary team.
Death certificates and statements of fitness for work.
Provide life support where necessary, such as during anaphylaxis or an asthma attack
Complete regular audits to improve internal systems and outcomes of care as part of the Quality and Outcomes (QOF) targets system. Attend and participation regular practice staff meetings.
